West Forrest Personal Care Home was established in 2005 located at 280 West Forrest Street in quiet, beautiful , historic district of Harlem, Ga. 30814. Angela and Joe Lokey are the owners and operators of West Forrest Personal Care Home . Angela worked for McDuffie Law Enforcement for many years and State Board of Pardon and Parole and Joe worked for Temple Inland for 35 years before retiring. Angela has always had a desire and compassion for the elderly and disable even as a small child. Joe’s mother suffered from Dementia for many years and Angela’s mother suffered from a battle with cancer and lost several sibilings at a young age due to health problems. Angela and Joe thought it was the right time for giving back to those in need for the elderly and disable. God blessed us both with ability to make our dream come true and opened many doors to full fill the desires of Love, Compassion and Caring for those in need. We are licensed by the State of Georgia Health Care Facility Regulations and are inspected at random. We are Medicaid Providers for two State Medicaid Programs Community Care Service Program and Source Program . We accept Medicaid clients who are on either of these Programs and we also accept Private Pay and Long Term Care Insurance. We are licensed for the capacity of 12 clients Private and semi- private.
